Healing Surges per encounter

Are really people doing it? (attacking players during short rest) Kind of defeats the idea of having per-encounter system in first place.

Depends on the DM I guess. I don't do it regularly, but I also don't like to give the players a false sense of security, so I'll sometimes do it if the situation warrents it, like the example James provided.

If the official adventures created by Wizards are anything to go by, short rests are meant as nothing more than lulls in the combat. That is, they're supposed to be so short that no reactions have time to take place.

Even in the case of adjoining dungeon rooms...

Attacking the party before they've had a chance of catching their breaths really suggest you're looking at the 5 minute figure too literally... A better definition would be "the short time from one wave of enemies to the next".

Myself, I try to be reasonably clear on when an encounter is "over". When an encounter is "over", you will have had time for a short rest before the next one by definition. Otherwise, the encounter wasn't "over" after all, and then I as the DM would not have indicated this.

This saves on wasted paperwork, where players start to edit their sheets only to have to change it back again just because the DM continues the fight.

In other words, I don't see any value in making the players believe they can have a short rest only to have monsters jump them "before its end". In this case, it would be much better and faster to just say "no you can't rest, let's continue acting round for round"...
