Help with a Giant Leader

A Cloud Giant has an attack bonus of +22/+17/+12. If this giant were to become a 9th level fighter, do I just add the fighter’s attack bonus of +9/+4 for a total of +31/+21/+12. I’m unsure. If I follow the rules for multiclassing, the giant should have an initial attack bonus of +31, for the first attack but I’m not really sure what the rules are for his extra attacks.

You add the BAB like a fighter who gained barbarian levels would. In this case just add 9 to all of the Cloud Giant's attacks, and grant him another attack.

Total should be:

+31/+26/+21/+16

Giant and Class Levels

While Technik4's solutions is the fastest way it might not be the most accurate.
You need to take the Giant's attack bonus of +22, decrease his Str modifier from this, and add a bonus equal to his size penalty. This will give you the giant's BAB. You then add +9 to it, and detemine his number of attacks from it, adding his size penalty and Str bonus to figure his attack bonuses.

Now, if you have the Epic Level Handbook you might want to use the rules there in cases when the BAB exceeds +20, and treast all extra fighter levels as Epic levels. In which case he is also eligible to take epic feats.

Well its simple enough. If the Cloud Giant has 3 attacks and it gains enough class levels to gain an attack with the new class (+6/+1) then his 3 attacks become 4.

Although this is imprecise, it is a fast way to do it for high level monsters. For medium level monsters, you would have to reduce the Strength -> Determine BAB -> add all BAB to determine #of attacks as normal.
