The 16 dex is more than you will need. You would be better served dropping it lower and increasing your con. FullPlate only lets you use 1 point of it. If you are not going the full plate route, then you will have a very tough time at higher levels since your AC will cap much lower than it would as a plate class and there is no need to not be a ranger or something.

so in the next campaign i'm playing an elf fighter (sword and shield)
 

i realize that 16 dex isnt min/max'd optimized "best" for a sword and shield fighter
he will wear a masterwork breastplate at 3rd level (the campaign starts at 3rd)
the character is not a turtle with a sword/shield
the character is a rabbit with a sword/shield

has anyone ever used the Dive for Cover feat?

I've never used it, but it only helps for reflex saves and I hate being prone.

I've also not had much luck with dex fighters in 3.x.
 

Pick up combat expertise, improved trip, and improved disarm. Trip works for those critters without a weapon, and disarm works with those critters with weapons. Eventually, you can pick up knock down (don't know if complete warrior has it, but sword and fist does).
 

When you get 2nd level spells, False Life should get a high priority.

Many people overlook this spell since it wasn't in the 3.0 PHB.

nice spell, but i'm a fighter, not a wizard, and the target is "you"
 

Why a 14 Int? Consider making it a 13 Int, 11 Con, so that you can put your level 4 ability mod to get more hp, or make it a 12 Int, 12 Con so you can put your level 4 ability to qualify for Combat Expertise.
