I voted "occasionally," but that's because it isn't always a group of monsters that I have waiting in the wings to attack the party. I have a handful of other tricks, like natural hazards or special items, that I will drop on the party from out of the blue when things start getting slow.
Battle works great for perking up a sluggish game. But I think that combat can be over-used. So in addition to the spontaneous battle (I heart wandering monsters), I will also mix in something interesting...
...like an interesting item (the body of an unlucky adventurer, who happens to be carrying a map of a section of the very dungeon the party is exploring, with perhaps a riddle or secret door scrawled on it to help guide the party to something they missed)...
...or something dangerous (fire on the mountain! Quickly, we must move to higher ground to escape the choking smoke and flames!)...
...or something flashy but benign (a tremor shakes the cavern, causing a harmless shower of pebbles and dust to fall from the ceiling.)
Combat is fun, but it's not the only way to perk everyone up.
