They're actually three separate questions.
Lightning Recovery lets you reroll your attack roll with a +2 bonus when you miss with a melee attack.
With concealment, you might miss with your attack. This triggers the maneuver - reroll your attack roll with a +2 bonus (which might end up being less than what you rolled the first time), and if that beats the AC, roll the miss chance. (Even though your attack roll was successful, and it was only due to the miss chance that your attack missed, Lightning Recovery doesn't permit you to retain your attack roll and only reroll the miss chance.)
With Mirror Image, you don't miss with your attack (assuming your attack roll was good enough); you hit your target. It's just that your target is not what you were hoping it was. Since your attack didn't in fact miss your target, Lightning Recovery doesn't trigger.
Incorporeality doesn't provide a chance for the attack to miss; rather, it provides a chance for the creature to avoid the
damage from the attack. Since the attack hit but dealt no damage, you have not missed with a melee attack, and Lightning Recovery doesn't trigger.
