Low Magic Magical Item Economy

I haven't really had a chance to read much 4e (though I'd love to try a game someday), but I've read that it handles item rewards and magical items much differently from 3.x. That made me curious about how other DMs & players running "low magic" worlds handle magic items, especially weapons and armors, in their campaigns.

I'm not really talking about purchase/sale thereof; I know that's a well-trod path. Personally, I will not allow much trade on the open economy of magic stuff for cash in my current campaign, because I think it doesn't fit the flavor I'm going for.

But how do you handle the 'combat economy' of magic in the game? Do you still give, somehow, your PCs +5 flaming vorpal longswords so they can handle elite supernaturals/outsiders? Or do they not receive those weapons, but still fight the default CR enemies? Or do they fight at a lower-adjusted CR (and do you have rules for determining that CR?)? Do you mimic the effect of magic via materials instead, or through some plot (the bebelith can only be wounded by a silver longsword with the hair of a virgin martyr forged into the blade!)?
