Well, humans are usually male or female but hermaphrodites have actually existed as well as many other conditions that would count as gender anomalies. With the life cycle of the mindflayer being the taking over of another creatures body of just about any humanoid (IIRC), I can only imagine there are plenty of chances for things to "go wrong". Then, you could always have a mind flayer with a twist who conducted experiments to become female or at least features of such, especially if it identified with a gender or found it helpful. They are prone to body grafts (again IIRC).
