Well, I actually use a modified Plane of Mirrors in my homebrew. I even resolved a character background plot point by referencing it. :eek:

In my version, you can only cross between two linked mirrors, and your reflection (ala Mirror of Opposition) steps out of the one you enter from. Neither of you has a reflection until one is "slain" -- and that one is trapped back in the Plane of Mirrors.

So, for my world, I'm going to have to add an ability like:

No Reflection (Su): Mind worms have no reflection, and pass through the Plane of Mirrors without creating duplicates.

time to work on the flavor text, then. :) hmm, you have no italicized descriptive text!

From the waters of the underground lake rises a creature that might be straight from a madman's nightmare - a huge wormlike creature, resembling a purple worm, but sickly pale and a bit smaller. Even now that its upper body has been out of the water for some time, it is still dripping, as though the water was seeping out of the creature's pores.
 

Shortening up and shifting the flavor text a little:

From the water rises a creature straight from a madman's nightmare - a huge wormlike creature, sickly pale purple in color. Even out of the water its upper body still drips, as though the water was seeping out of the creature's pores.

The mind worms are beings that can attack and swallow creatures through any reflective surface. A mind worm has the same general appearance as a small purple worm. Thought to be an experimental creation of the mind flayers, mind worms have great mental capabilities and some psionic abilities. In addition, they can create so-called “probe worms”, which can attack opponents near a reflective surface at any distance, even on a different plane. Due to this ability, illithids, who are credited with creating the mind worms, have used them as assassins and bounty hunters.

Combat
A mind worm prefers to assault opponents with its probe worms rather than confronting them directly, attacking obvious spellcasters first to drain them of their life energy before being swallowed by the probe worm. When confronted on its own turf, a mind worm tries to lure its opponent into the water where it has the advantage.
 


OK, time to work on the psionics. Taking what I found in the original file, we need two separate blocks, something like this:

Psionics (Sp): At will—greater scrying (DC 21), plane shift (DC 21). Effective caster level 15th. Save DCs are Charisma-based.

Psi-Like Abilities (Sp): At will—remote viewing (DC X), psionic plane shift (DC X). Manifester level 15th. Save DCs are Charisma-based.
 



Not sure about the DCs though, shouldn’t that be 10 + Cha mod (4) + level?
So for remove viewing that would be 18? And psionic plane shift has no save, does it? Would be 19 if it did, apparently.
