It's in Tome of Battle. +4 to your effective monk level for the purpose of calculating your unarmed damage. If you're not a monk, there's a table you use instead. Requires BAB +3 and Improved Unarmed Strike.

It's nice. Finally gives non-monks a way to do decent unarmed damage.

Sounds like Supreme Unarmed Strike is substantially the same as the Monk's Belt, and would therefore not stack.

But do either or both of these not have a maximum of your current level or HD a la Practiced Spellcaster?

That's all the feat says on the matter.

They're similar to caster level increases. In this case, both the belt and the feat are a) in different amounts, and b) from different sources. No reason they wouldn't stack.

Nope, neither have any such limitation.
