The biggest issue would be the destruction of neverwinter, of course they could set the game during the spell plague or centre it around some reclamining of Neverwinter.

Setting it during the spell plague would make the most sense to me. That way they could use the 4e rules but still use the story and characters from the 3e era. All the important characters from the novels and such would still be around to interact with, but the beginnings of the spell plague would cause enough havoc to explain any minor hiccups that conflict with the overwhelming amount of FR lore.

If they did go with the full-on 4e era, I assume it would revolve around the rebuilding of NW, but that doesn't make much sense. MMOs love the draw upon existing characters that people know. It makes the NPCs feel like more than random faces with loot/quests.
