There's a really cheesy "Kung Fu Action Theater"-style flick called Chinese Super Ninjas which does actually depict a fairly realistic ninja organization/operation, when it's not being silly. The "Chinese super ninjas" are the heroes who get all kinds of special training to defeat the more-or-less realistic (within context) ninja villains.

'cause, y'know, it was made in China, so obviously the Japanese guys gotta be the villains. :\

Here's the lowdown:

IN REALITY: Ninjas were commoners who were displeased with the government's disarming them, which is why they learned to fight with a myriad of ordinary things used for agriculture: such as Grain flails (Nunchucks), Scythes (Kama), and Bo staffs (long pieces of bamboo). If they had to go over a wall, they carried a ladder. if they had to steal something, they took it when nobody was looking. They were effective because they did simple "stab and kill" assassinations and could disappear just by bowing their head or putting on a different hat.

Those weapons were developed because commoners were forbidden to have weapons, but the ninja families did not start thier use.

Ninja are effecient. In D&D they should use every trick in the book to deliver CDG's. They should not be a fun foe to fight. The only listen check you should get to hear them coming is at -10.

I don't. When ever I see someone who is in their 20s or 30s wearing Naruto gloves or a Naruto bandana walking towards me, I tend to ignore them or walk the other way (because even I do not need that sort of company).

You can't succeed on a spot check against a ninja. The ninja only exists when time has come to strike, he is not really hidden anywhere before then.
