They are considered the same entity nowadays.I did not have one on my computer of her in her dragon form.
Besides, people might mistake her for tiamat in that form, and I wanted to keep the two separate entities

Gods can have different manifestations on different worlds. A true death on one would just result in permanent banishment from it.Doesn't make a lot of sense considering in Dragonlance Takhisis dies but in Forgotten Realms (or Baator) she's alive and imprisoned, but the DMG does indeed say in a magic item's description that Takhisis is Tiamat's name on Krynn.
