That's something that kept throwing me off with Dragonlance for a long, long time.

(Of course, DL has its own problems, such as the fact that Ansalon is tiny.)

Gonna put in some of the landscapes one of my too be 4e games will employ:
  • Huge cities that dot the world, their tallest skyscrapers visible even on the edges of the horizon. The cities are built on massive ruins that litter the landscape, the cities themselves thus starting hundreds of meters above the ground.
    The ancient ghettos (called the Nest) and the factories that churn up energy from hidden Ley Lines and Nexuses form the foundation of the city and rising from it hundreds of skyscrapers and raised train-tracks and walkways.
    The city is lit by electrical lighting and subtle use of reality fluxes to create areas of light. Moving advertisements and 3-dimensional displays also are powered by reality fluxes. Cables, wires and pipes the feed energy and other necessities crisscross the city and especially the ghettos where many homes are built around the massive pipes or are crushed by new ones.
    The tallest skyscrapers are owned by the Aristocracy, Nobles and the Clergy, to have a watchful gaze on the city.
    Dirigibles, ships and trains are the most common ways in and out of these cities.

  • Black Monoliths and ruins are strewn across every surface of this Artificial World, their origins are unknown but no mortal hand could create structures so huge.
    It is common for cities to be built atop them and towns around them, one famous city was built into one, a tower rising high into the sky. It’s Dirigible docking points spreading out, and from afar seeming like only hairs against the size of the tower.

  • There are storms that crisscross the world, they are powerful wandering masses of uncontrollable reality, the energy generated by the countless realities created within them cause massive storms to form around it. The common elements of these storms can vary for each storm but they include; lightning and combustion of creatures/objects within reach of the storm.
    Some storms last months at a time slowly drifting across the world, others appear randomly, though usually triggered by extreme use of reality-flux magick. The uniting aspect of both types of storms is their aftermath besides the physical carnage caused by the energy surrounding them they also warp and mutate most of the landscape and what creatures lay within it.

  • Ley Lines and Nexuses are less obvious transformations of the landscape and are most noticeable by Dirigible. However the effects the cause can be wide spread. They generally warp the landscape somewhat, Ley Lines causing roads, trees, rivers, etc. to follow the same path of itself and with Nexuses causing a spiral appearance; this is most evident in stone circles.
    The actual effect the reality that seeps out of Ley Lines and Nexuses cause certain reactions to objects or people, generally the most common aspect is to give unusual properties to one. So a rock finds itself levitating, or a revolver’s bullets turn into an acidic mass upon contact with something, etc. Many Reality-Deviants and those who can use reality-flux magick gained such from spending time within these zones.
This is only a taste of what different locations my campaign world has.
