Why was it controversial? I'm thinking I probably ran it way back in the depths of time, but I don't really remember.

The original version (by Jean Wells) had some art that seemed to be mocking the TSR leaders, plus some otherwise racy art (for the time) and rather odd monsters. Stories vary a little on what it was that so enraged TSR leadership - but it was recalled and destroyed.

The module that was widely released was rewritten extensively by Tom Moldvay.
