Plane Shifting to the Material

I.e. do you just make it "5d100 miles off course on the world's surface, roll 1d12 to pick a clock-face direction"?
If I have a specific campaign map, then basically, although I'd probably be using a 1d4 (N, S, E, W); without a specific campaign map, then I roll for what terrain they land in - Forest, Desert, Lake, Ocean, field, swamp.... whatever I think of at the time.

No. I tend not to play 2nd edition's insta-death on bad teleports....

Once I had the PCs appear by an erupting volcano and they had to avoid the lava flow and get away. Another time I had them appear near the Styx. Good thing they didn't drink from it.
