In brief, it puts a 14 HD cap on the spell and bans Polymorph into Constructs, Outsiders, and Undead (unless you are already of the type). Also you can only increase in size to one more than your current one.
Disorientation is eliminated, but so is changing multiple times.
The spell has become Monomorph Other.
