My first "real" (i.e., played more than once or twice) PC was a female fighter (well, still is, I still play her occasionally). As the campaign progressed, she married another party member, and got pregnant.
We had a big gaming weekend that happened to coincide with Olivia being pregnant. As we were figuring out which PCs we were going to play (most of the players in the campaign have more than one PC), the DM dangled a carrot: if you play Olivia throughout the weekend, she'll get enough XP to make 10th level (this is back in 2E days, when that was a lot of adventures worth of XP).
So, Olivia (about 3 months pregnant at the start of this) had some "maternity studded leather armor" made, with expandable gussets. She managed to survive (though was operating at a Dex penalty by the end), as did the baby.
