Lets say you cast Heat Metal on someone then that someone teleports away. Does this end the spell or can you still concentrate on it continue to damage the spells target?

PHB 203: "Once a spell is cast, its effects aren't limited by its range, unless the spell's description says otherwise."

So, yes, you can continue concentrating and the spell will keep dealing damage (until the target rids itself of whatever metal object you're heating, or you hit the 1-minute duration limit).
