The newest book, Heroes of the Forgotten Kingdom, has some interesting feats that help with Combat advantage as well. In addition to reprinting distant advantage, there is cunning stalker (c/a if the target is not adjacent to anyone, although you are exempted if you want to melee them), hidden sniper (concealment is enough to give you c/a with ranged attacks) and terrain advantage (if opponent is in a square of difficult terrain, it provides you with c/a)

Are those Rogue-only, or available to everyone? Either way, wow. It seems to suggest that WotC really wants you to be able to get CA without flanking, so long as you are willing to pay for it. For example, Hidden Sniper should give most Warlocks CA just about every turn so long as they use Shadow Walk effectively.
