So if a Dragon initiates a grapple as a free action after biting and succeeds dealing bite damage again and then next round breaths shouldn't the character get a reflex save?
I wouldn't allow a save. If someone is in a dragon's mouth, they are basically dead meat anyway. Even as a player myself I would realize that and just start writing up a new character. I should know, everytime I come up against a dragon my character dies horribly.
Though I understand that you do not get a save in this situation, according to the rules, this can't be applied universally. I'm afraid that if a DM simply denied me the opportunity to fight, simply because they think that I can't win, I would have to kick him in the jimmy.
Besides, never underestimate the lethal effectiveness of a swallowed character armed with a multi-empowered Cone of Cold wondrous item in the belly of a Red Dragon.

Perhaps you come up against them too early. Perhaps you're ill-prepared. Perhaps your party doesn't use the best tactics. Perhaps your DM likes to throw Great Wyrms against 10th level characters to prove some kind of sick, twisted, immature point.
I disagree to a certain extent. I agree with the example you were responding to, but there are other cases where I think DM adjudicating in player favor is called for.
