Which methods of representing afflictions in D&D do think work best - or which do you least-dislike?

I chose all of them. I like a wide variety of effects from afflictions so as to maintain a bit of mystery in the world and reflect the nature of real afflictions. This also helps to make afflictions non-trivial because We as Players will suck the fun out of the game by developing an efficient solution to afflictions.
