Can also go a little crazier and give him a boon that works like Elven Accuracy (1/enc reroll) , Deva Memory of a Lifetimes (add 1d6 to a roll), or Heroic Effort (+4 on a miss) or a theme like Harper (+1d6/enc) or Pack Outcast so he has combat advantage more often.

Frankly, if you can get away with it, you can throw him a Harper Pin pretty easily and that might solve your problems.

In my personal experience on my fighter, I will say that I almost never miss. I get CA more often than ranged, have +3 prof, +1 from class, and have a couple backup abilities to make it less likely.

I'd look at:

Combat Advantage, Expertise, Luckblade (reroll attack as daily), in paragon or high heroic, Fortune Stones (in this case, probably Stone of Earth [melee weapon only] (another reroll)), Potions of Clarity (these are mostly too expensive, since sometimes you won't use the reroll, but as treasure...), and Lucky Charm (daily d6 to add to an attack). And the aformentioned Avenger multiclass (which combines really, really well with reroll powers); note that Discipline is just better than Hero of the Faith; the only advantage of HotF is that you can have both.

That said, another approach is to select powers that aren't as affected by a miss or are less likely to miss. This is true for most dailies, but for first level fighter encounter powers, that's:

Shield Riposte: Immediate Action, so if it misses the player still gets a standard action attack on his or her next turn. [effectively, it's a multiattack]

Hack and Hew: Multiattack, so if it misses with one attack it might still hit with the other. [con: attacks two targets]

Passing Attack: Another multiattack

At higher levels, he can pick up close burst attacks that again, end up acting as multiattacks where a single miss doesn't mean he's completely wasted the power.

Learning weapon is a good idea, though.

For more flavour make it an intelligent weapon, that has the spirit of a legendary swordsman trapped within it. Play it up as the master noting the habits of the opponent, telling the Fighter how to exploit them. It could also be used as a plot hook, later on.
 

Has he otherwise maxed out his bonus to hit? That is, is he using a +3 proficiency weapon, have at least an 18 to start with in his primary stat, maximizing opportunities to get CA, waiting for the leader/controller to buff/debuff, etc.?
 

I'm assuming you know this by level 9, but all Daily powers do something on a miss. Either they are Reliable, do half damage, or have a Effect (or, multiples of these).

I try to choose Daily powers with good "miss" effects, just in case the dice gods hate me for that particular attack roll.

Not all of the level 9 dailies have something on a miss - Victorious Surge is reliable, but gives you nothing on a miss. Same with Thicket of Blades. (missing two times in a row with a single daily would be even more frustrating!) Terrifying Impact is not reliable and does not do anything on a miss (though, it has an effect that grants a second attack.) Piquing Dare is not reliable and does nothing on a miss. Same with One Against Many, Murderous Assault, Bonecrusher and Fighter's Recovery.

It's not that his character isn't good on paper - he just has terrible luck rolling. Having the expected "to hit" for your level doesn't help if you roll a "3" and a "4" to hit with your daily, and then a "5" with your Encounter.
 

There's also another tack that he could take, to increase his damage output and accuracy. There are a number of feats that increase accuracy and damage with opportunity attacks, like Blade Opportunist. If he can't hit reliably with his main attacks, then he can improve his ability to punish people under his mark.
 

Can you post the build - there are few (non-custom) items that do what you want and for very good reason.

I don't have his exact build, but he's a greatsword fighter, human. I'm pretty sure he's maxed out in STR. So, 18, plus +2 for Human, +1 for level 4 and +1 for level 8 for a 22 STR. I think his current greatsword is a +2.
