When a player was falling and low on hits he just started drinking potions he had hoping one of them would help, which they did, but in the process one of them was acid.
I had him make a Fort save which he failed. So I thought I would add a slight extra penalty, but I wondered for how long and asked for another save. He failed that as well. Well I lowered his Cha 2 points (random 1d4) and also made him have to make concentration checks for verbal spells as the damage was that extensive, unless he had the time to take a 10, which in combat he did not.
William was a Low level Wizard so it did really impinge on his character but it didn't kill him, made him, max out his concentration skill. He has since been healed but he conmtinues to max out his concentration skill, even though now in combat it's over 20.
I would never kill a player for something like what happened here, but some sort of long term effect is always a challenge for players and characters.
If they knew they were drinking acid and did it anyways maybe some thing harsher but I still wouldn't kill them over it.
