A player is about to go up against a small cult of disease-worshipping cultists - I was wondering if anyone had any ideas for interesting disease-themed beasties I could throw in? Note that they need to be low CR...
Rats/Dire Rats are good for disease. So would certain of the undead. Many natural animals could be used to spread diseases through their bite attacks, the wounds if not properly cleaned could go septic.
Goblins, goblins and more goblins. Festering puss-filled yellow-toothed disease ridden goblins. Just have them attack with their claws and successful hits cause Fort saves. You could call them... well.. goblins. or Fester Goblins (they are short and bald after all). or The Diseased.
If you have time you could invent Plauge Elementals (MoP has some guidelines for this type of thing) for low CR, just make them smaller. (Small Pox)
Or you could have Diseased Undead, depending upon the flavor of the cult. Same as the goblins, just have sekletons or zombies that are so diseased you make a Fort save when they damage you.
I would suggest the Vermin Lords from Creature Collection 2. The highest CR of them is 3....
In fact, I would suggest purosing the entire CC2 - there are several interesting Disease carriers in there. I do not have it in front of me or I would give you specifics.
That's right in CC1 there is one that pops to mind. A guy so filled with disease he looks undead. Using this is funny because your clerics keep trying to turn him. They finally cut him down and ask, "Why is the undead thing bleeding?"
Morgaunts (those are the disease ridden types that are not undead), Touch Corrupturs, Chosen of Vangal, Verminplagues. There's plenty to choose from in BOTH CCs.
