If a PC was fighting a housefly, by all means he has the Swallow Whole extraordinary ability.

Although, there are many better ways to kill a fly. Ugh. Just imagine the taste from that.

I'd argue that a medium-sized creature could easily swallow something whole...provided it was Diminutive or Fine. A human being couldn't swallow a Stirge whole any more that one can swallow a cat.

The Extraordinary special ability doesn't imply that only larger creatures can swallow things whole. It merely represents the fact that a Gargantuan creature is capable of doing that due to their proportion to the PCs. If a PC was fighting a housefly, by all means he has the Swallow Whole extraordinary ability.

I disagree. Without the (ex) ability Swallow Whole, I don't think you can use this ability in combat. On a helpless Fine opponent, sure! But that's different than combat.

If Swallow Whole was a matter of size difference rather than a special ability, I imagine it would be listed in the PH as a special attack form. Also, if it was just based on size difference, why don't all Gargantuan and larger creatures have it listed?

Imagine that,

She swallowed a cat,

She swallowed the cat to catch the bird,

She swallowed the bird to catch the spider,

She swallowed the spider to catch the fly,

I don't know why she swallowed the fly,

That is quite clearly not what an Extraordinary ability is. Of course you're free to use that as a house rule, but it contradicts the rules as written.

What is special is they don't chew. So if you want to swallow whole it has to be about he size of a polo or a pea.

Does anyone else think it odd that creatures can swallow you while your still alive? I mean, who would do that and how silly is it to have 100hp left and still be hacking away at something on the inside. Even more rediculous is with successful grapple checks you can crawl out again without causing any serious combat detriment to the monster.
