Teleport vs dimension door in combat.

I'd call a place you actually see "very familiar" for Teleport. 94% On-Target, 3% Off-Target, 2% Similar Area, 1% Mishap.

Reasonably safe. Let's say you teleport 30 feet. You'll be 3 - 30 feet off target in a random direction. Pretty safe, usually.

Similar area? That's a DM call for sure, but it seems lke

"You wind up in an area that’s visually or thematically similar to the target area.

Generally, you appear in the closest similar place within range..."

Implies some place pretty darn close when only going 30 feet.

Even a "Mishap" is not too bad, just 1d10 damage and a 1 in 20 (5%) chance of another mishap.

As for Cape of the Mountebank. That works because only the Raven cannot act any more than round. That's really funny - the image of a Raven with a teeny, tiny cape on.

Unfortunately, the Raven will likely not survive for long.

That's contrary to the description in the teleport spell. It explicitly says, with my emphasis,

Teleport said:
“Studied carefully” is a place you know well, either because you can currently see it,...

What do you see that implies that? The only "range" that this could be referring to is the spell's travel range. That isn't affected by the distance you attempted to travel.

Even if you try to teleport within the same room, a Similar Area result could send you up to 100 miles away per caster level.

Generally speaking, if you are teleporting a few feet (or even a few tens of feet) within a room, the next nearest similar place will be another spot in the same room, most likely. Sure, it's possible the next nearest, similar is pretty far away, but it's unlikely.

BTW: Anyone notice the chart is backwards. High numbers are supposed to be good results in a d20 game. :)

Somewhat on topic: with two feats (Least Dragonmark/Passage from Eberron CS and Quicken Dragonmark from ?Magic of Eberron?) you could get the ability to do short-range self-only combat teleports (10ft/level) as a free action. I have a tank with both of those feats (as well as Lesser and Greater Dragonmark so I can do it 3/day) and I absolutely adore the greater mobility of the instaports. I think I've used Dimension Door once in the entire campaign, and I have yet to use Teleport--but I'm constantly hopping all over the battlefield like an insane frog on steroids.

I've never regretted taking those feats. : )
