How do you handle hat of disguise? Does it change your appearance as the spell for as long as you wear the hat or is it limited to 1st level casting time?

The item is priced at 1800 gp and may be used by anyone. Looking at the prices for wonderous items, we find that allowing a first-level spell to be cast at will costs 1800 gp. Making the spell continuous (for as long as one wears the hat) involves a higher cost.

In short, either one is possible, depending on how much your character spends.

Hmm. Seems from the MM that most monsters with SR actually add any class levels to their "base" SR to get a final value. That's true of all the ones that have "monsters as PC" writeups at least. Does it follow that monsters that don't have such writeups (like demons) also get to add class levels to their innate SR?

I haven't looked up this rule, but I would think if it is specified in some and others it just provides a flat rating then the ones not specified would not have it advance.
