Tiamat has had a very close relationship with the Dark 8 (who effectively act as Asmodeus's proxies in the Blood War), up to and including having a child (Mordukhavar the Reaver) with Cantrum the deceased (supposedly) founder of the Dark 8. The abishai are effectively on permanent loan to Tiamat by the grace of the 9 Hells for her service of guarding the passage between Avernus and Dis. 5e (as a result of 4e) may have screwed majorly with the continuity of course.

The Lore as of 3e is unchanged there. The only difference is who is on Loan to who. The Abishai are Tiamats Creations in 3e and she lets the rest of the hells use them. (As they are made of the same stuff all other devils are, they can be promoted into alternate forms of Devils as well.)

Not according to the article about her in Dragon 350, which places the first abishai as a collaborative work between Tiamat and Pearza of the Dark 8.
