To add my 2 cents worth

Magical flying is flying with the aid of magic (duh), and in the case of the fly spell (or fly like spell effects) solely by the aid of magic. So no, if you need to flap your arms to fly then you are not going to have much luck underwater.
Can a person with the fly spell fly in
1 - mist? - yes
2 - rain? - yes - reduced movement rate - if you dont want to fly into things
3 - a deluge - yes (see above)
4 - in space - yes (basically no air)
5 - underwater - why not? I dont see little turbine engines popping out my shoulder blades every time I cast fly.

And in the rules where it states you get full movement whilst in the air it does not state ONLY in the air, so a penalty for moving through more viscous environments is perfectly in order. And no, it does not mean I can now burrow at my fly speed, because the burrowing movement rate is about how fast you can DIG! Just in case a very important point is being overlooked this is MAGICAL flying, with no flapping of arms or cloaks required. Since I am old school when 3rd Ed does not supply a concrete answer I call back on 2nd Ed. which supplied a hell of a lot more detail. Flying works underwater but with movement penalties depending on what you are moving through, if you can't walk through a rock all, you can't fly though a rock wall. I can walk/swim through liquid water so surely I can fly through it. What if you ended up in an environment that was so cold the air froze. Could I fly though frozen air... NO.
