Use Magic Device and Mishaps

When using the skill Use Magic Device (p. 75-76, PHB), does the mishap apply only when trying to activate an item blindly, or when failing any attempt to activate a magical device (such as emulating alignment, race, ability score, etc)?

Looking at the way the Players Handbook presented the skill, the mention of the failure penalities appears in the activate blindly section, so I'm not quite sure if it applies to the entire skill or simply that aspect of it.

The SRD has the text laid out in a better way. The final paragraph talks about the mishap, whereas all the options are in seperate paragraphs which are not broken.

This would indicate that the mishap occurs at all times if the roll is 10 less than required.
