I'd rule that the percentage change is based on the original default Medium size.

Correct. The *chance* of hitting hasn't changed. But 400' is further than 200' which is further than 100'. Which is why I think the *range* increments should change. The chance that any of your examples can hit each other is not effected by the weapon size.

Unless of course a Small being tries using a Large weapon. That wouldn't go far to my mind. I'd apply a range increment penalty plus the attack and damage penalties already reflected in the rules.
