The only time that one of my PC's has died was when another Player was controlling the character at the time. All the characters were only 1st-level, and a lucky goblin scored a critical, which incidentally brought her to exactly -10 hp. I fudged the rolls and made it -9 instead and "forgot" about losing hp from dying and stabilization.

I didn't think it was right for a character to die because of such crappy luck at such a low level, especially when the Player wasn't even there.

I had a player get killed by a ghoul after rolling three natural 1's in a row on Fortitude saves...how embarassing. Then my poor dwarf bard got reincarnated as an elf.

wow, a fate worse then death.......

Once a player of mine got double criticaled by a kid using two daggers and died. It was meant to be a diversionary encounter, nothing important, but there were some really good dice rolls. We were agape, but looking back on it, it is one of the most hillarous stories around the gaming table. I wouldn't have it any other way. :)
