What happens when you use the Hide skill?

Oddly, I can't find a definition for "Hidden" in the SRD. I've always treated successfully hidden characters as effectively Invisible for purposes of attack bonuses and whether the defender is denied Dex. But the rules don't back that up.

Does anyone have a PHB, SRD, or FAQ entry that covers hiding / how a hidden creature is treated with regards to attack bonus and the defender's Dex bonus?

I treat it as invisible except individuals can negate the effect by making a spot check with DC=your hide check.

Yeah, me too, that's how everyone does it. But there's no rule in the SRD (that I can find) to support the practice. Maybe there's something in the PHB?

I don't think so. Hide is such a nebulously defined skill - the designers probably didn't want to be any more specific than they had to be.
