One problem is that the snake's swiftness spell explicitly says that it doesn't stack with haste. "This spell does not allow the subject to make more than one additional attack in a round. If the subject has already made an additional attack, due to a prior casting of this spell, from the haste spell, or from any other source, this spell fails."

Ok... Monk with ITWF, Quick Draw, and Rapid shot could make... 6 attacks without assistance, 2 BAB, 1 Flurry, 2 off hand, and as long as any of the previous attacks were with a shuriken (or other ranged weapon a monk can use) 1 ranged attack.

If you have Haste that's another attack, Snap Kick adds another. If you use Roundabout Kick (free attack if you make a critical with an unarmed strike) and have Improved Critical you could potentially get several other attacks.

If you're not restricting it to melee only, the Master Thrower's Palm Throw can give you a ton of attacks. Rapid Shot + TWF + ITWF + Flurry + Haste + 2 from BAB all times two is 14 attacks. They're all low damage attacks with no strength bonus, but things like sneak attack, divine favor, a friendly bard, and divine might can alleviate that. If you're a halfling they'll almost always be touch attacks too. You can tack trips on as well, but I wouldn't do it until the end of the flurry because if they fall they get +4 AC against the rest of your thrown weapons.

There's also spells you can add on for more attacks, like the Evard's Menacing Tentacles in PHBII or the various animal attack powers in the XPH. Add the tentacles and Bite of the Wolf to that thrower and you get 16 or 17 attacks (can't remember how many the tentacles give).
