I just got the book yesterday. I have read a good bit of it, but I haven't had time to think on it very much.
As for what I would do with it:
I am not running a game now. When I get around to running a new game, I will use it to design the A-list npc's of the game.
It will be fun to introduce epic elements into a game from the start. Not that the players will face anything epic, or if they do, they wont know it. Though for the myths and legends on the world, the ELH will be put to good use.
I do think that I will limit some things. Epic spell development is great, as it is an extension of things that characters can already do. I will even allow some epic skill use. Anything that borders on the fantastic, I will allow. Strait out, impossible for a mortal, I won't allow. For example: Walking on Clouds. As a skill use, I go for something like this. If the party wants to dance around in the upper branches of a bamboo forest..well yeah, okay. but walking on Clouds....no way.
If characters in this future game ever reach the epic level, I will use the option that something in game has to happen to cross the "epic threshold". Strait exp wont get them there.
I plan on using the optional rules for combined skill use, massive damage, and expanded death thresholds from the start of the game.
