I never bothered to check, but I always assumed that it was in the main errata as well as being in Tome & Blood. Right?

Nope.

In 3.0 we had an official polymorph in the first printing of the PH, the PH errata (for the second printing), Dragon Magazine, the Psionics Handbook, Tome and Blood, the Psionics Handbook errata and the SRD. All different, although some of the differences are minor.

Not to mention the new wild shape writeup in Masters of the Wild.

(Don't ask me for the order they appeared or the exact differences. It's been a while since I went over the various versions and unless you really, really, want to know I don't want to do the research again. Oh, and the second PH printing may have had its own slightly different variant too, but I'm not sure.)
