Skill and Powers and Combat and Tactics for 2nd Edition. People gripe it was ridiculously overpowered (and it was), but I love the options in class/race building. Great stuff.
 

Universal Brotherhood for Shadowrun.
Creative layout and presentation. Awesome plot twists. Great suspense.

I'd also give a nod to Hunter the Reckoning: The Walking Dead for many of the same reasons.

My favourite supplement ever is the 3.0E Forgotten Realms Campaign Setting. Great amount of stuff crammed into a beautiful package. I'm hoping Ptolus will top that, though.

Planescape - it made D&D (well, in those days AD&D) interesting again
I'll second that. I ran a Greyhawk-Planescape game for probably 10 years and it was definitely the most fun I've had playing D&D.
