For 4e my monsters have to-hit and defences as if they were in the range +2 to -2 compared to the PCs. If a monster is higher in level, it gets to-hit/defences as if it was +2 levels higher than the PC's, and vice versa for low level monsters. The same goes for skills.
It's my implementation of bounded accuracy for 4e. I came up with it after reading about 5e and thinking that bounded accuracy really sounded like what I have been missing to run a more simulationist game. I sort of wanted to run 5e, but I really think it needed a couple of more iterations and content before I would want to use it.
