Rule Natural 20 on Death Save, no gaining a level in Exhaustion, thx
Roll of a natural 1 on Death Saves and you will gain 2 points in Exhaustion, if you manage to stay alive that is

This happens all the time "likely to be thrown back into the fight with 1hp left" Retreat to a safe location and shoot Arrows

Sounds okay but understand it will have an effect on game play, probably slowing down the party. Since you can only shed a single level of exhaustion per Long Rest the effects can be very hard to shake unless adequate rest is taken.
 

Someone on reddit had this as a house rule and I think I will add it to my campaigns soon.

If you get two failed death saves you gain a level of exhaustion.

I like it, simple and has repercussions for being so close to death.
 

Just be careful about repeated recovery. A character lucky enough to roll a 20 on stabilization is likely to be thrown back into the fight with 1hp left, and can quickly pick up a second level of exhaustion.

Could be easily ruled that when you have a level of exhaustion, you can become stable, but cannot get back up at 1 hp.

with that long Rest, if you use the Medicine Skill check (DC 15) with a dose of Healing Kit or dose from the Herbalism kit that you could make a house rule that it reduces the Exhaustion level by one more
