Zone of Truth

How does this 2nd level cleric spell really work?

Saving Throw: Will negates.

As I read it, if a creature fails a save, he knows he is affected and most speak the truth or evade answering the question, but he can not lie.

If the creature makes a save, he can still lie.

Does the caster know if the target creature made his save against the Zone of Truth? Or can a creature that saves lie and nobody will know?

Just as it is possible for a person to be in the area of a spell and not know it (such as a precast zone of truth) it is possible for a person to resist the magic of a spell and the caster never realize. Unless specifically listed, a caster never knows how well his spell worked.

Although this remains true for Zone of Truth, that's an incorrect statement. A caster always knows if his target makes or fails their saves when casting a Targeted spell, but if it's an Area or Effect spell, he has no idea.
