Can a character become hidden without using the hide action?

The Stealth rules in 5e are a mixed bag. On the one hand there are simple. Everything is left to the DM to decide. Then again, there are few hard rules which makes determined stealth/hidden a freaking mess. I used a lot of old 3.5 stuff for stealth in 5e.

Stealth and the Art of Hiding
• You cannot Hide from a creature that can see you (requires heavy obscurement). To successfully hide, your opponent either doesn’t know that you are there, is not paying any attention to you, or doesn’t know where exactly you are located.
• Hide Action. When you try to Hide, make a Stealth check vs. a targets passive Perception. If you are successful, you are Hidden (but not necessarily Unseen).
