The world of John the Balladeer, as envisioned by Manly Wade Wellman. The atmosphere is perfect for an rpg, and southern gothic Appalachian weirdness has not been successfully captured in a ttrpg yet, imo.

I love SWADE but I don't think it is a good fit for Elder Scrolls. I would want something definitely crunchier. But I agree about the not-5E part. Maybe if not 2d20 it would be a good candidate for AGE or even BRP?
BRP, or its relatives and descendants, seem like a good starting point to me. Especially if you consider the "do it to improve it" approach you find in the games. Would probably need quite a bit of work to make it feel adequately high fantasy.

I am a bit surprised no one has yet asked for an RPG set in the John Wick universe. For good measure, one could get John Wick (the designer) to contribute.
I think 2LM's Outgunned is as close as you can get without an official license. If someone were to slap the right names on things, I'd say they'd be done within weeks.

We have gotten 2 huge Appalachian Weirdness games in the last year.

I suggest checking out Dungeon Crawl Classics #83 The Chained Coffin for Dark Appalachian weirdness fantasy.

There is also more directly the Old Gods of Appalachia RPG using the Cypher system and based off the fiction podcast.

And I just found Holler: An Appalachian Apocalypse by Pinnacle Entertainment for Savage Worlds.
