There's actually a 3.5 edition adventure called "Fires of Hell" that is played when the PCs die as a way to give them the chance to "fight" their way back. Someone converted it to 5E. I haven't played it, but I have read through it and seems really decent. So, you could try that. Might be...

I was thinking about the extra damage issue and how to handle that, i.e. whether to give advantage on the damage roll (like some are suggesting) or a flat damage or something else. I was thinking that it might be useful to take the KIND of mount into consideration. I mean for a mounted warrior...
There's a homebrew archetype (yeah, mine, ha ha) for Strength based rogues that gives Medium armor as a bonus proficiency, which get's to your AC issue.
