It doesn't. Those aren't the only possible extraordinary sacrifices. But, for a very physically oriented character like a barbarian, they are simple and obvious places to start when considering what an extraordinary sacrifice would be.
For example, clearly a barbarian giving up his spellcasting ability would not be an extraordinary sacrifice. It wouldn't be a sacrifice at all. A barbarian fighting or killing for someone might be making a sacrifice, but it is hardly extraordinary - in fact, it's a very ordinary sacrifice. Depending on the culture, even giving his life for someone might not be considered extraordinary.
For something to be extraordinary, it has to be something that most people would not or could not make themselves do. It has to show that they are committed to the Exalted path above all else.
