AOO and reach

I need to clarify AOO's

If a char moves in on a monster with reach 10" then the monster gets an AOO as the chars moves in to close?

If a monster had reach 15 would the monster (with combat reflexes) get two attacks??

What about a char moving around a monster within its reach can they get a barrage of attacks?

Yes; no; the monster would get one AoO.

You can only make on AoO per round unless you have combat reflexes. Even if you do have combat reflexes, you can only use one AoO on one opponent once per round. So if a player is fighting a monster with 15 ft reach, he moves in and suffers an AoO. The player then can cast a spell while standing next to the monster without making a concentration check (casting defensively) without suffering an AoO, given that there are no other foes within reach. Combat reflexes merely makes it possible to make AoO against multiple opponents.
