Okay, I am hoping someone can help me out on this one.

His only limit on the number of times per day he can assume a bear form is the number of times per day he enters a rage or frenzy,...

Any bear warrior can assume the form of a black bear once per day.....
At 5th level, a bear warrior can assume bear form twie per day and choose between black and brown bear forms.

At 10th level, a bear warrior can assume bear form three times per day and can choose between black, brown, and dire bear forms.

Is it just me or do the second two quotes completely contradict the first quote? Is it limited by the times he can rage/frenzy per day or by the form he can take?

It looks like hold over text from the OA Bear Warrior to me. The OA Bear Warrior gained additional bear form uses when it gained access to higher forms. I do not think that the CW Bear Warrior is meant to do the same.

I was thinking the same thing until I noticed than their exemple of a Bear Warrior character was also limited in the number of uses per day of its Bear Form.

The CW's Bear Warrior exemple character can Rage 2/d and can use Bear Form 2/d. Now this could be either because he has two Rage uses per day and so he has two Bear Form as well or because because he is 5th Bear Warrior level he can use his Bear Form two times per day as per the entry I believe to be in error. It is not clear which is the case here.

There is a contradiction in the rules (unless the Bear Warrior's Rage uses per day equals the Bear Form use limit [as in the example]) either a Bear Warrior can use his Bear Form ability number of times per day equal to the number of times per day he can enter a rage or frenzy or he can only use his Bear Form ability 1/d at 1st, 2/d at 5th, and 3/d at 10th. I can not ascertain with certainty which is correct. I am inclined to think that the limit is a hold over from the OA class. In any case this ability needs some clarification.

The example character is a Bbn 7/ BW 5. That means he can only rage twice per day. So I am guessing that is what is limitng his bear forms per day as well. So the first quote is the correct one and it is limited by number of rage times per day and not the level of bear forms.

I would think that the number is tied to rages. My main reason for this is that the chart doesn't mention a limit. Almost always when WotC has limited uses tied to level they add it to the chart for the PrC.
