Ninety degrees straight down. Some (few) medieval crossbows had little prongs on them to hold the bolt down. For the rest, the user kept his thumb on the quarrel.

In D&D there are things that can walk on ceilings with cross bows which is why I wondered

I wonder how different firing an ordinaty bow upside down feels?
