If a creature is deafened they suffer a -4 penalty to intiative as per the definition of deafness in the PHB. My question is does this -4 take effect even after initiative has been rolled?

For example if a PC rolls and initiative of 20 and is the deafened is his initiative reduced to 16 until he can hear again or does it remain 20?

You don't change init in the middle of a fight, except as a result of actions like delaying or readying. For example, if you took temporary ability damage which reduced your Dex bonus, you wouldn't recalculate your init.
