When a will save is available. But when there is no will save, and instead it says 'willing only', they do not.
OK, this is an old thread, but I'm glad I had it marked and came back to it. I had previously thought that it really was the unconcious are always willing, and thus don't get a will save, I personally prefer this.
Now for potential abuses... first level spell Benign Transposition. Two willing targets switch places. Verbal only, close range. Take a bucket of puppies (from the amuzing great cleave thread), keep them unconcious, and you can always get out of a grapple even at first level!
PoP -> They're now holding a cute little puppy. (If it's a particular half orc from Order of the Stick, you'll not only have escaped, you'll have also completely taken it out of combat!)
