I will fudge for the PCs or NPCs alike when I feel it appropriate for the long term enjoyability and stability of the campaign and my players' enjoyment of the game.
 

I almost never fudge die rolls, but I will fudge monster tactics to try to prevent player deaths at lower levels. Just stuff like spreading my attacks around, when it's clear that if the monsters focused on one target that they could take it down.

I generally don't bother with these sorts of fudges at mid to high levels, when PCs are more resilient and these kinds of challenges seem less unfair.

Cheating. As in, saying the critical hit you rolled for a monster was a normal hit, or saying the damage put the PC at -7 hp instead of at "dead".

I try to run a "clean" campaign, but the temptation to spare the PC's can get pretty strong.

I try not to cheat in favor of the bad guys -- I never make a miss a hit for them, for instance. However, I will sometimes "modify on the fly", when my notes are messed up or I meant to right they had X spell or item but forgot to write it down.
 

It'd be kind of interesting to have a balanced fudging system for DMs. For every fudge a DM makes in his favour, he must give a fudge of proportional magnitude in the players' favour at some point that session.
 

Sometimes I fudge some rolls during a combat, mostly because I made a wrong decision and choose to strong or weak encounters. So sometimes I max the HD during the fight to make a fight more challenging.
 

I voted a 5. I fudge rolls now and then, almost always to ensure an encounter runs smoothly. If the party are having too easy a time of it then the monsters will get in a couple hits to remind them not to get too cocky. If the party have their backs to the wall, then a hit might well become a miss or the damage dealt will be a little less. I never fudge a fumble (1) or a critical though.
 

I haven't fudged a roll since I introduced an action point mechanic last year. However, I do occasionally fudge monster stats if I planned badly (generally by taking away or giving HP).
 

I almost never fudge, but when I do, the most common one is "This monster has +10 hp." One of my players does wicked damage with those twin Dwarven Waraxes, and suffers from terrible "post-monster letdown" when the critter doesn't last at least three rounds.

I voted a 6. I'll fudge a couple rolls a night, usually in the party's favour and for story reasons. As DM, my job is to let the players have fun and tell a good story, not to be a slave to the (arbitrary and self-imposed) rules.
